This morning as I was walking pass a clinic to get some breakfast from a kopitiam next to it when I saw a mother holding her two children by the hand, leading them into the clinic entrance glass door. The children looked flushed, and were sniffling – so I thought, these children must be sick. I peered into the glass door, and I saw a 2-3 children in the clinic, assuming that they are sick as well. I made a conclusion, probably it’s the heat and the unpredictable weather.

The hot weather is really something you can’t avoid. You can’t tell the sun to stop radiating and you can’t rearrange the clouds to shade the Earth from the scorching heat, right? Here’s what you need to do to keep you and your family cool and healthy during this crazy hot weather in Malaysia! Here are the top 5 ways to keep you and your family healthy!

No. 1 – always keep you and your family hydrated. Drink plenty of water throughout the day, and not only when you feel thirsty. Because your body lose water through excessive sweating, you’d end up dehydrated and parched like a dried prune. Occasionally, you can have a glass of cool peppermint tea. Ovarian Cyst is Not Fibroid

Is ovarian cyst similar to fibroid? No. It is not. Though it occurs in the same section of a woman’s reproductive system, it is two totally different thing. Ovarian Cysts are fluid-filled sacs that form in or on the ovaries. One type of cysts – a functional cyst – occurs quite often. Usually, ovarian cysts do not cause symptoms and go away fairly quickly without any treatment. But in some cases, large cysts may cause pelvic pain, frequent urination, and other symptoms. In that case, you should consult a doctor who will recommend appropriate treatment.

Other types of ovarian cysts can represent tumors in the ovary. They can be benign or malignant. A doctor can diagnose this type of cysts with an ultrasound, and blood tests can be done to tell if the cyst is malignant. Such cysts are treated by surgery.

Most ovarian cysts are never noticed and resolve without women ever realizing that they are there. When a cyst causes symptoms, pain in the abdomen or pelvis is the most common one. The pain can be caused from:

  • rupture of the cyst,
  • rapid growth and stretching,
  • bleeding into the cyst, or
  • twisting of the cyst around its blood supply (known as torsion).

If the cyst has reached a large size, other symptoms may arise as a result of pressure or distortion of adjacent anatomical structures. These other symptoms can include

  • abdominal fullness, expansion of the abdomen, or bloating,
  • low back pain
  • indigestion
  • feeling full after eating only a small amount (early satiety),
  • urinary urgency
  • difficulty with emptying the bladder completely,
  • feeling an urge to defecate
  • having difficult bowel movements, or
  • pain with sexual intercourse.

There are many causes of ovarian cysts, and most ovarian cysts are not cancerous. Sometimes ovarian cysts may be noticed by a doctor during a bimanual examination of the pelvis. If a cyst is suspected based upon symptoms or physical examination, imaging techniques are used. Most cysts are diagnosed by ultrasound, which is the best imaging technique for detecting them. Ultrasound uses sound waves to produce an image of structures within the body. Ultrasound imaging is painless and harmless. Transvaginal ultrasound is a diagnostic tool to better visualize the ovaries using a thin ultrasound probe inside the vagina. Other than that cysts can also be detected with other imaging methods, such as CT scan or MRI.

What about treatment? Well now, the ideal treatment of ovarian cysts depends on the likely cause of the cysts and whether or not it is producing symptoms. The woman’s age, the size of the cyst, and the cyst’s appearance on ultrasound help determine the treatment. Cysts that are functional are usually observed with frequent monitoring unless they rupture and cause significant bleeding, in which case, surgical treatment is required. Benign and malignant tumors require operation.

Treatment can consist of simple observation, or it can involve evaluating blood tests to help determine the potential for cancer but ovarian cancer is rare in women younger than age 40. After age 40, an ovarian cyst has a higher chance of being cancerous than before age 40, although most ovarian cysts are still benign even after age 40.

Fibroid. It seems like it is everywhere on social media, health group chats, health group communities; we, especially women, are beginning to be aware of this issue. But, what is fibroid?

Is it dangerous? Can it lead to a much worse fate? No worries loves! Here it is, all you need to know about fibroid,

First, before you can cure something, you will need to know what it really is. Fibroid, its most common name is also called Uterine fibroid – is a non-cancerous tumor. Yeap, it’s a kind of tumor. It grows from the muscle layers of the womb, which growth can vary from the size of a green bean to being as large as a melon. Scientifically known as leiomyomas and myomas – it commonly affects around 30 percent of all woman by the age of 35 years old, and 20 to 80 percent by age of 50 years. These benign growths of smooth muscle usually develops in women ages of 16 to 50 years – which are the reproductive years where estrogen levels are at its highest.

It is still unclear of as what brings about fibroids except for the fact that they start developing when the estrogen levels in a women’s body is at its peak! Most people experience no symptoms, but they can include lower backache, constipation and excessive or painful uterine bleeding that leads to anemia – loss of blood. Other possible symptoms may also include, labor problems, pregnancy problems, infertility and repeated miscarriages. If fibroids are large, there may also be a lot of unexplained weight gain and swelling in the lower abdomen. Once fibroid develops, it can continue to grow until menopause – and shrinks once the estrogen levels falls after menopause.

There are apparently four types of fibroid. The classification of a fibroid growth depends on its location in the womb, check these out:

  • Intramural: This is the most common type. An intramural fibroid is embedded in the muscular wall of the womb.
  • Subserosal fibroids: These extend beyond the wall of the womb and grow within the surrounding outer uterine tissue layer. They can develop into pedunculated fibroids, where the fibroid has a stalk and can become quite large.
  • Submucosal fibroids: This type can push into the cavity of the womb. It is usually found in the muscle beneath the inner lining of the wall.
  • Cervical fibroids: Cervical fibroids take root in the neck of the womb, known as the cervix.

Fibroids usually does not any symptom, but they are usually caught while doing a routine pelvic examinations. Some diagnostic test can detect the developments of fibroids to rule out other conditions. These diagnostic test includes:

  • Ultrasound images by scanning over the abdomen or by inserting a small ultrasound probe into the vagina.
  • An MRI can determine the size and quantity of fibroids.
  • A hysteroscopy uses small device with a camera attached to the end to examine the inside of the womb. The device is inserted through the vagina and into the womb through the cervix. If necessary, the doctor can take a biopsy at the same time to identify potentially cancerous cells in the area.
  • A laparoscopy may also be performed. In a laparoscopy, the doctor makes a small opening in the skin of the abdomen and inserts a small tube with a lighted camera attached through the layers of abdominal wall. The camera reaches into the abdominopelvic cavity to examine the outside of the womb and its surrounding structures. If necessary, a biopsy can be taken from the outer layer of the womb.

Course of treatment – well in most cases, fibroids can be left alone and untreated – if the fibroid does not affect the quality of life, treatment may not be necessary. Also, fibroid can lead to heavy periods, but if it does not cause major problems, one may choose not to undergo any type of treatment. This is because, as explained in the first three paragraph, fibroid will shrink when a woman hits menopause due to low estrogen level. Although, when treatment is absolutely necessary, first point of treatment is, medication and or surgery. The location of the fibroids, the severity of symptoms, and any future childbearing plans can all affect the decision of treatment.

Some severe cases where fibroid causes the following and should be taken seriously because it may be life-threatening:

  • Menorrhagia, also called heavy periods: This can sometimes prevent a woman from functioning normally during menstruation, leading to depression, anemia, and fatigue.
  • Abdominal pain: If fibroids are large, swelling and discomfort may occur in the lower abdomen. They may also cause constipation with painful bowel movements.
  • Pregnancy problems: Preterm birth, labor problems, and miscarriages may occur as estrogen levels rise significantly during pregnancy.
  • Infertility: In some cases, fibroids can make it harder for the fertilized egg to attach itself to the lining of the womb. A submucosal fibroid growing on the inside of the uterine cavity may change the shape of the womb, making conception more difficult.
  • Leiomyosarcoma: This is a rare form of cancer that is thought by some to be able to develop inside of a fibroid in very rare cases.

But if all is well, and no severe symptoms can be detected due to fibroids, first thing you can do is maintain a healthy lifestyle. By keeping your weight in check, and ensuring that you have proper exercise and healthy diet can help you maintain a healthy estrogen level.

  • Uterine fibroids. These noncancerous growths in the wall of the uterus can cause pain.
  • Adenomyosis. The tissue that lines your uterus begins to grow into the muscular walls of the uterus.
  • Pelvic inflammatory disease. This infection of the female reproductive organs is usually caused by sexually transmitted bacteria.
  • Cervical stenosis. In some women, the opening of the cervix is small enough to impede menstrual flow, causing a painful increase of pressure within the uterus.

Some other factors that you might be at risk of menstrual cramps if:

  • You’re younger than age 30
  • You started puberty early, at age 11 or younger
  • You bleed heavily during periods (menorrhagia)
  • You have irregular menstrual bleeding (metrorrhagia)

Hello beautiful people! How are we today? I have a question for all of us, have you ever had that sudden pain on only one side of your head after receiving a bad news? And I bet you feel like you are gonna suddenly collapse and that the end of you. Well, some may call it a headache, but that kind of piercing pain that causes a hissing or buzzing sound in your ear, you feel like your eyes are going to pop out if the pain doesn’t stop – I introduce to you the culprit that contributes to all your misery and lack of interest in tolerating any kind of annoyance, The Migraine!

Like many things, the first ever migraine was discovered and recorded by the ancient Egyptians and date back as far as 1200 B.C. Migraine is one of the oldest ailment known to mankind. Much much later into human civilization, around 400 B.C – ish, Hippocrates, a Greek physician of the Age of Pericles, referred to the visual disturbance that can precede a migraine such as flashing lights or blurred vision, which we know as aura. Hippocrates also describe a person will feel relief after vomiting.

The word migraine was derived from the older language in the world – Latin word “hemicrania” which means “half” (hemi) “skull” (crania) – basically migraine means half skull, given that often times migraine only occurs on one side of the head. Galenus of Pergamom – also a Greek physician, surgeon and philosopher in the Roman Empire was the first to use the term, “Migraine” to describe the pain felt across one side of the head during a migraine. Galenus suggested that the pain is likely to originate in the menenges – a layer of are three layers of protective tissue called the dura mater, arachnoid mater, and pia mater that surround the neuraxis; and the vasculature of the head. He also pointed out the connection between the stomach and the brain due to the vomiting that seemed to relief migraines.

Migraines often start early in life of a person – be it during their childhood, adolescence phase or early adulthood. This ailment may or may not progress through four stages : prodrome, aura, headache and post-drome.

Prodrome : One or two days before a migraine, you may notice subtle changes that warn of an upcoming migraine, including:

  • Constipation
  • Mood changes, from depression to euphoria
  • Food cravings
  • Neck stiffness
  • Increased thirst and urination
  • Frequent yawning

Aura : May occur before or during migraines. Most people experience migraines without aura.

Auras are symptoms of the nervous system. They are usually visual disturbances, such as flashes of light or wavy, zigzag vision. Sometimes auras can also be touching sensations (sensory), movement (motor) or speech (verbal) disturbances. Your muscles may get weak, or you may feel as though someone is touching you.

Each of these symptoms usually begins gradually, builds up over several minutes and lasts for 20 to 60 minutes. Examples of migraine aura include:

  • Visual phenomena, such as seeing various shapes, bright spots or flashes of light
  • Vision loss
  • Pins and needles sensations in an arm or leg
  • Weakness or numbness in the face or one side of the body
  • Difficulty speaking
  • Hearing noises or music
  • Uncontrollable jerking or other movements

Sometimes, a migraine with aura may be associated with limb weakness (hemiplegic migraine).

Attack – Migraine usually lasts from four to 72 hours if untreated. The frequency with which headaches occur varies from person to person. Migraines may be rare, or strike several times a month. During a migraine, you may experience:

  • Pain on one side or both sides of your head
  • Pain that feels throbbing or pulsing
  • Sensitivity to light, sounds, and sometimes smells and touch
  • Nausea and vomiting
  • Blurred vision
  • Light-headedness, sometimes followed by fainting

Post drome – The final phase, known as post-drome, occurs after a migraine attack. You may feel drained and washed out, while some people feel elated. For about 24 hours, you may also experience:

  • Confusion
  • Moodiness
  • Dizziness
  • Weakness
  • Sensitivity to light and sound

In younger humans, the telomeres are about 8000 – 10,000 nucleotides long and shortens as each cell continues to divide. Nevertheless, when they reach a critical length, the cell just stops dividing and dies. Therefore, the processes of cell death and cell aging are regulated in part by telomeres.

Aging shorter telomeres gives a negative effect on our overall health. For example, telomeres protect cell’s chromosomes from fusing with each other or rearranging – an abnormality if were to happen can lead to cancer because without the telomeres to protect them, cells will be destroyed. Shorter telomeres also increases the chances of cells to deteriorate and producing molecules that can lead to inflammation, which is another important risk factor for various diseases. This is where aging starts to take its effect on the human body because telomere shortening is the primary cause of age-related break down of our cells. This occurs because when telomere length reaches a critical limit, the cell undergoes, in a scientific term – senescence and/or apoptosis – which basically means the cell will age or deteriorates and will soon dies! Basically, to put it in a simpler term, as the telomeres gets shorter, you age quickly and the risk of inflamed or age-related diseases is higher.

With every year, your telomeres get shorter, some cells stop replicating, and these symptoms worsen. UV and metabolic damage, which further shorten telomeres, can trigger cancer and aging; when you break the plastic cap, you unravel the whole shoelace.

Today, I am going to share something interesting which is CP500.

What is CP500?

Why CP500?

Everytime when I received something from LHDN, my heart beats faster than usual. Do you feel the same as I do? LOLZ!

As an income earner in Malaysia, we cannot runaway from paying tax. Every year, we need to declare our incomes and expenses so that we know what is our taxable income. We need to pay income tax to the government based on the taxable amount.

Here is the chart that you may want to keep as record.

This is my very first year receiving my love letter, CP500 from LHDN.

What is CP500?

CP500 is what we call as Income Tax Installment. It is a notice of Prepayment of Income Tax by Instalment.

CP500 normally will be sent early of the year to those who are ‘selected’ where you have to pay your (estimated) tax on an installment basis.

Who are the people who will be receiving this CP500?

Normally CP500 is given to people who has an income but not from their fixed pay. For example, people who drive GrabCar, people who has extra income via house rental or car rental, people who are doing online business and anyone who has a source of income but not paying PCB monthly.

Since I am not working now and I am receiving my monthly bonus from Shaklee, so I have to pay for this Prepayment of Income Tax by Installment via CP500 Notice.

This is actually a form of initiative to help tax payer to pay for their tax on a bi-monthly basis instead of a whole lumpsum during the tax declaration period.

If case the income tax amount is lesser than the amount that you have paid under this installment period, government will refund back to you as usual.

How Does LHDN Decides on the Amount on CP500?

LHDN estimates the tax amount based on the income tax paid in previous year.

For example, I am paying my tax of RM6000 in Year 2018. So LHDN will issue CP500 with an estimated tax of RM5000 and divide them into 6 months.

I have to pay my CP500 of RM1000 every bi-monthly starting March 2019 until January 2020.

How to Pay your CP500?

You can pay at most of the banks in Malaysia over the counter for CIMB Bank, Public Bank, Malybank, Affin Bank, Bank Rakyat, RHB Bank and BSN. You can also pay via online transfer using CIMB, PBB, Maybank2u, HLB, RHB, Vitibank, Alliance Bank and BSN.

You can choose to pay via LHDNM website at http://www.hasil.gov.my using FPX method.

If you prefer to walk in to LHDNM, you can choose to pay using cheque, bank draft to Ketua Pengarah Hasil Dalam Negeri.

If you choose to pay via CP501 Form, make sure you fill your details properly on the form which is attached together in the CP500 Notice and send to Pos Malaysia Berhad. The details and address is included together in the CP500 Notice.

Penalty for Late Payment

YES! There is always a penalty is you fail to pay on the “Tarikh Akhir Kena Bayar”, last date to pay. A penalty of 10% up to 100% will be imposed for late payer. So make sure you set your alarm every 2 months to pay for your CP500. Hehe.

So, don’t worry!

The government is not collecting extra money from you. It is just a system for you to pay your tax in advance via their installment program. It is common among those who are working with a company because they have to pay for their PCB (Monthly Tax Deduction) every month as stipulated in their payslip. It is a series of monthly deductions that go towards payment of your taxes in relation to your employment income. These monthly deductions are retained by your employer and paid over to the Inland Revenue Board (LHDN).
